ABSTRACT
Background and purposeAnatomical changes may compromise the planned target coverage organs-at-risk dose in particle therapy. This study reports on practice patterns for adaptive therapy (APT) to evaluate current clinical wishes barriers further implementation.Materials methodsAn institutional questionnaire was distributed PT centres worldwide (7/2020–6/2021) asking which type of APT used, details workflow, what implementation were. Seventy from 17 countries participated. A three-round Delphi consensus analysis (10/2022) among authors followed define recommendations required actions future vision.ResultsOut 68 clinically operational centres, 84% were users at least one treatment site with head neck being most common. mostly performed offline only two online (plan-library). No centre used daily re-planning. Daily 3D imaging by 19% users. Sixty-eight percent had plans increase their use or change technique APT. The main barrier "lack integrated efficient workflows". Automation speed, reliable deformation accumulation higher quality in-room volumetric identified as urgent task APT.ConclusionOffline implemented majority centres. Joint efforts between industry research clinics are needed translate innovations into feasible workflows broad-scale
