Rapid evaluation of T cell clonality in the diagnostic work-up of mature T cell neoplasms: TRBC1-based flow cytometric assay experience

ABSTRACT
The identification of mature T cell neoplasms by flow cytometry is often challenging, due to overlapping features with reactive cells and limitations currently available clonality assays. description an antibody specific for one two mutually exclusive receptor (TCR) β-chain constant regions (TRBC1) provides opportunity facilitate the detection clonal TCRαβ+ based on TRBC-restriction. Here we prospectively analyzed 14 healthy controls 63 patients protocol used suspected neoplasm implemented immunostaining targeting TRBC1. Specimens were firstly classified in 3 groups clinical records data, laboratory findings immunophenotypic features. was assessed TCR Vβ repertoire analysis new rapid TRBC1 assay. Results showed that unimodal expression unequivocally associated samples presenting aberrancies. Moreover, demonstrated use useful solving uncertain cases confirmed high sensitivity method identifying small clones significance (T-CUS). Finally, found a degree concordance (97%) comparing assessment methods proposed method. In conclusion, our results provided real-life evidence utility introduction cytometric evaluation routine diagnostic work-up neoplasms.
