ABSTRACT
Aspirin-exacerbated respiratory disease (AERD) is a difficult-to-treat syndrome where timely diagnosis and initiation of disease-specific therapies are pertinent to improved patient outcomes.To characterize the most common timeline for development clinical triad [asthma, nasal polyposis, reactions nonsteroidal anti-inflammatory drugs (NSAIDs)], identify barriers prompt AERD, describe indications an aspirin challenge facilitate accurate diagnosis.Six hundred ninety-seven patients with diagnosed AERD history at least one sinus surgery remove polyps were identified in Brigham Women's Hospital registry. Patient reported age onset asthma, first NSAID reaction obtained from 2013 2019 enrollment.Of 697 identified, asthma preceded polyposis reaction, although there was considerable variability between patients.Prompt important provider education care this population patients. Consider diagnostic without historical NSAIDs who have otherwise compatible history, specifically take daily low-dose aspirin, leukotriene modifiers, avoid NSAIDs, or severely symptomatic baseline it would be difficult acute worsening symptoms.
