ABSTRACT
Historically, age has been considered to be a relative contraindication for organ donors. The use of elderly donors liver transplantation remains controversial due the fear inferior outcome. According United Network Organ Sharing (UNOS) data, proportion older increasing annually. This study describes short‐ and long‐term outcomes donor livers. Three hundred seventy‐four primary transplantations, which had performed at University Virginia Health System from 7 February 1988 31 December 1998, were included. Graft survival, incidence non‐function, hepatic artery thrombosis (HAT) after according different groups analyzed. Cases analyzed by (group I, n=106: aged <20 yr; group II, n=217: between 20 49 III, n=51: ≥50 yr), in comparison with recipient IV, n=65: recipients transplanted organs within 5 yr their age; V, n=266: donors> younger than VI, n=43: age). Group III or VI advanced age) II V (control group) compared age, gender, race, body weight, height, pre‐transplantation cytomegalovirus (CMV) status donors, cause brain death total warm ischemic time, ABO matching, degree human leucocyte antigen (HLA) mismatching. No significant difference graft survival was found (p=0.604) (p=0.567). Moreover, no differences non‐function HAT age. Older more likely women have antibodies CMV, as well died cerebrovascular causes. Race, height both time grafts, HLA mismatching not significantly groups. We conclude this that is if careful assessment made on case‐by‐case basis. There need maintain an open mind regard livers current situation serious shortages.
