ABSTRACT
Abstract Continuous access to electronic health records will fuel the digital transformation of medicine. For data-sharing initiatives, challenge lies in ensuring data aligns with interests holders. Federated authorization, where remains controlled locally, may offer a solution balance these interests. This paper reports on implementation federated authorization system used German National Emergency Department Data Registry. Using from 2017 2024, we analyzed system’s effectiveness managing nationwide research network 58 emergency departments. Facilitating more than 7.9 million records, 75% queries were authorized within 15 days. The also supports periodic queries, enabling recurring real-time access. Query volumes grew over 23,000 by completion rates 86%. thus serve as blueprint for initiatives worldwide.
