ABSTRACT
Abstract Primary tumor resection (PTR) is recommended for patients with unresectable stage IV colorectal cancer (CRC) who present symptoms related to their primary tumor. However, the survival benefit of PTR asymptomatic controversial. We investigated change in rates and contribution CRC over past two decades United States. Clinicopathological factors long-term were compared 44 514 diagnosed from January 1, 1988, through December 31, 2010, had or not undergone PTR. Multivariable Cox regression instrumental variable method used identify independent survival. Of CRC, 27 931 (62.7%) The annual rate decreased 74.4% 50.2% 1988 2010 median overall increased both non-PTR patients. Instrumental analyses revealed that was associated better overall, cancer-specific other-cause CRC.
