ABSTRACT
We aimed to investigate the value of maternal serum delta neutrophil index (DNI) levels in predicting placenta accreta spectrum patients with previa. The who were found have previa totalis included our study. Location placental implantation and depth myometrial invasion defined by transabdominal transvaginal 2D gray scale Doppler sonography confirmed during cesarean section histopathological evaluation. Patients subjected complete blood counts, including prenatal hemoglobin level, total white cell count, differential leukocyte platelet count. The following formula was used calculate DNI level: (%)=(leukocyte subfraction analyzed cytochemical reaction MPO channel) - (leukocyte using nuclear lobularity channel reflected light beam measurements).Placenta detected 295 patients; 31 them had PAS. As control group, 189 evaluated. In group PAS, significantly higher (p<0.05) than other groups. only also group. univariate model, a significant effect number sections observed separating multivariate independent (p>0.05) Significant efficiency [area under curve 0.899 (0.814-0.984)] differentiating efficacy 5 cut-off 0.858 (0.770-0.946)] distinguishing PAS (sensitivity 80.0%, positive predictive 64.9%, specificity 91.6%, negative 95.9%) CONCLUSION: Maternal values seem be beneficial respect both prediction. Although more comprehensive studies are needed test this proposition, prediction practical should done different trimesters its usability preventing maternal-fetal morbidity investigated.
