Recently, endoscopic submucosal dissection (ESD) using the pocket-creation method (PCM) has been developed as a kind of third space endoscopy. PCM enables complete under lesion before making circumferential incision [1]. Using this method, wide should be created with small entrance where small-diameter endoscope is more desirable.
