“Four I” Framework for Telehealth Optimization in Congregate Care Communities

ABSTRACT
Background: Telehealth can provide innovative models of care for people living in congregate communities (CCC), but lack consistent workflow is a barrier administrators and staff. We propose framework CCC to implement workflows age-inclusive telehealth. Methods: As part an infection control initiative with focus on telehealth optimization, Virginia Infection Mitigation, Prevention Control Through Technology developed relationships staff across the Commonwealth Virginia. Partners this community practice completed statewide survey that we conducted anticipated experienced barriers implementation. responses, virtual meetings organizational leadership, on-site facility visits, our team assessed strengths, needs, goals capability. Working administrative clinical teams, consultation report define short- long-term implementation steps. Results: collaborated nonprofit organization supporting neurodevelopmental disabilities rural Program All-Inclusive Care Elderly. optimization four tiers: Initiate, Integrate, Incentivize, Inspire.
