ABSTRACT
Background and aims: Cardiac tamponade is a rare complication of systemic lupus erythematosus (SLE) especially in the pediatric population as an initial presentation. We report thirteen years old young girl who was admitted to emergency department with severe chest pain, generalized edema, because cardiac plevral effusion, later diagnosed SLE. Aims: SLE should keep mind differencial diagnosis tamponade. Methods: She succesfully treated pulse methylprednisone cyclophosphamide medication together percutane pig tail catheter placement on pericardium rather than surgery. For percutaneous catheterisation, we advanced into by subxiphoid approach 0.035 inch guide wire through 7f. pig-tail parietal transthoracic echocardiographic guidance fluid drainage achieved. This had been prevent recurrent pericardiosythesis without need pericardial window surgerical procedure. Results: Pulse metylprednisolon (1gr/kg/dose) given two days cyclofosfamide 500mg/m2 weekly life treathing effussion. The child responded well treatment thus repeated X-ray picture improved, examination showing minimal amount fluid. discharged from PICU fifth day hospitalisation.
