ABSTRACT
Idiopathic achalasia (IA) is a chronic disease without definite therapy. Ethanolamine oleate (EO) has multiple biological effects, including inflammatory activities. We investigated the efficacy of EO injection in selected patients with IA.One hundred and thirty-six IA were evaluated prospectively. 13 that are resistant to or poor candidate pneumatic balloon dilation and/or cardiomyotomy at Digestive Disease Research Center, Shariati Hospital, Tehran, as major referral center for Iran an interventional study. Diluted was injected divided dose into each four quadrants lower esophageal sphincter, using standard sclerotherapy needle. Injection repeated 2 4 weeks after first injection. The symptom score (ASS) timed barium esophagogram (TBE) before injections. Good response defined decrease ASS least 50% baseline height volume TBE, 1.5 months last Side-effects recorded. All (13 cases) had good (decreased, ≥ 50%) TBE (decreased barium, rate. mean decreased from 11.38 (± 1.5) 3.23 1.96) (P=0.001). 81.38 ml 51.11) 40.69 61.22) (P=0.016). duration follow-up 17.83 1.12) months. Symptoms six relapsed; all them treated effectively reinjection. This study indicates well tolerated potentially effective might be explained by local properties EO. As presented data too preliminary support routine use treatment IA; its cases can considered.
