ABSTRACT
Organizational culture is seen as having a growing impact on quality and safety of health care, but its hospital to community patient discharge relatively unknown.To explore aspects organizational develop deeper understanding the process.A qualitative study stakeholders in process. Grounded Theory was used analyze data.In 5 European Union countries, 192 individual 25 focus group interviews were conducted with patients relatives, physicians, nurses, general practitioners, nurses.Three themes emerged representing culture: fragmented primary care interface, undervaluing administrative tasks relative clinical process, lack reflection process or improvement. Nine categories identified: inward providers, awareness needs, skills, work patterns professional counterpart, collaborative attitude, relationship between providing "here now" situation, considered be burdensome, negative attitude toward feedback, handovers at ruled by habits, appreciating integrating new practices.On basis data, we hypothesize that extent which providers value outreach critical effective discharge. Community often are insufficiently informed about outcomes. Ongoing challenges remain unspoken opportunities for improvement overlooked. Interventions address key factor efforts needed.
