ABSTRACT
Currently there is no global agreement as to how extensively a radical prostatectomy specimen should be sectioned and histologically examined. We analyzed the ability of different methods partial sampling in detecting positive margin (PM) extraprostatic extension (EPE)-2 pathologic features prostate cancer that are most easily missed by prostate. Radical specimens from 617 patients treated with open between 1992 2011 were analyzed. Examination entirely submitted detected only PM 370 (60%), EPE 100 (16%), both 147 (24%) specimens. determined whether these would have been diagnosed had examination limited alternate sections (method 1), representing posterior aspect gland addition one mid-anterior aspects 2), every section aspects, supplemented remaining ipsilateral anterior if sizeable tumor seen 3). Methods 1 2 13% 21% PMs 28% 47% EPEs, respectively. Method 3 demonstrated better results missing 5% 7% EPEs. Partial techniques slightly more EPEs low-risk intermediate-risk cancer, although even high-risk cases none all studied aggressive features.
