ABSTRACT
<p> </p> <p><strong>Abstract</strong></p> <p><strong>Rationale</strong>: Although patients with central disorders of hypersomnolence (CDH) exhibit characteristic symptoms hypersomnia frequently, it takes 5–15 years from the onset for its diagnosis due to lack symptom recognition. Here, we present a case idiopathic (IH), CDH, wherein early was aided by video footage spontaneous sleep attack.</p> <p><strong>Patient concerns</strong>: A 21-year-old man lost consciousness while driving and experienced an accident. He had complained excessive daytime sleepiness (EDS) over half year. During his hospitalization close monitoring loss consciousness, in-room surveillance camera captured 14-min long attack, during which he general muscle weakness without warnings or convulsions leading fall bed. There were no abnormalities in vital signs.</p> <p><strong>Diagnoses</strong>: significant cataplexy less than two sleep-onset rapid eye movements latency tests, mean 2.1 4.6 minutes. Other deprivation syndromes excluded differential finally IH confirmed according criteria Third Edition International Classification Sleep Disorders. course disease, attention-deficit/hyperactive disorder (ADHD) gaming also diagnosed. <p><strong>Interventions</strong>: Pharmacological treatment modafinil administered methylphenidate ADHD. Cognitive behavioral therapy performed disorder.</p> <p><strong>Outcomes</strong>: The EDS improved, attacks longer observed. disruption daily life caused reduced.</p> <p><strong>Lessons</strong>: Video recordings are beneficial identifying cause consciousness. Home may be helpful IH.</p>
