ABSTRACT
The pectoralis major musculocutaneous (PMMC) flap was once considered the workhorse for head and neck reconstruction; however, because of proliferation free tissue transfer, it has rightly taken on a secondary role. Nevertheless, in certain reconstructions, PMMC remains last-line treatment only salvage option do-or-die scenarios. conventional harvesting method cuts lateral thoracic artery all intercostals branches from internal mammary vessel to avoid compromising pedicle length. Nonetheless, dissection these 2 dominant sources blood supply skin islands overlying lower poses potentially high risk distal necrosis. To preserve vessels, is very valid choice viewpoint supply. In novel surgical procedure named "Supercharged Pectoral Major Musculocutaneous Flap"—"SUP-PMMC flap"—devised by us, vessels near bifurcation subclavian are cut then anastomosed cervical vessels. causes no vascular insufficiency compromise length part flaps. author used this technique 4 cancer partial necrosis or fistula formation observed.
