ABSTRACT
To determine rate and predictors of sphincter-preserving surgery (SPS) for rectal cancer patients treated at specialty institutions.SPS has been considered a surrogate surgical quality, sphincter preservation is tremendously important to patients. Evidence association between case volume SPS prompted recommendations that all undergo institutions. However, rates SPS, the factors associated with ability perform have not well-characterized.A prospective registry colorectal 7 National Comprehensive Cancer Network institutions was used identify clinical stage I-III undergoing (n = 674) September 2005 October 2007. Patient, tumor treatment were abstracted; patients' characteristics without compared using descriptive statistics multivariable logistic regression.Of 674 identified (median age, 58.2; 60% male), 520 (77%) had SPS. Of these, 240 low anterior resection coloanal anastomosis, 268 anastomosis; 12 other procedures. Sixty-two percent temporary diverting stoma. On analyses, independent included younger age diagnosis, proximal location in rectum, nonfixed tumor, institution.SPS exceed those seen population-based samples trials. In addition expected variation based on patient characteristics, we among Although optimal remains unknown, this provides areas further research potential performance improvement.
