ABSTRACT
To assess the feasibility and safety of robotic-assisted laparoscopic anatomic hepatectomy. The development minimally invasive surgery has led to an increase in use hepatectomy. However, hepatectomy remains technically challenging is not widely developed. Robotic represents a recent evolution that being used increasingly for complex surgical procedures. Herein, we report our initial experience with 13 consecutive patients.Between April July 2009, patients underwent hepatectomies benign malignant hepatic diseases. Major were performed 9 patients, left lateral sectionectomies 4 patients. Eight major diseases 5 (1 sectionectomies) All procedures anatomically hilum dissection. Prior starting parenchymal transaction, vascular control portal vessels was carried out whenever possible. These compared 20 traditional 32 open resections contemporaneous cohort-matched.All successfully manner pure resection. No conversion laparotomy or hand-assisted resection occurred. Despite its longer operative time (338 minutes) higher hospital cost ($12,046), robotic liver favorably blood loss (280 vs. 350, 470 mL), transfusion requirement (0 3 20, 32), Pringle maneuver 6 32) overall complications (7.8% 10%,12.5%). Neither ascites nor transient decompensation occurred group. The margins all 8 lesions negative as yet, no intrahepatic recurrences metastases have been observed mean postoperative stay shorter procedure (5.2 days) than (6.7 days)or (9.6 days). Conversions from 2 (10.0%) who right hemihepatectomy hepatectomy, respectively. These preliminary results show safe feasible much lower complication rate system may broaden indications hepatactomy, it enabled surgeon perform precise which required hylum dissection, hepatocaval endoscopic suturing, microanastamosis. more long-term, evidence-based outcomes will be necessary prove efficacy, further research on cost-effectiveness still required.
