ABSTRACT
Biliary atresia (BA) is the most common indication for liver transplantation (LT) in pediatric population. This study analyzed comprehensive factors that might influence outcomes of patients with BA who undergo living donor LT by evaluating largest cohort longest follow-up world. Between November 1989 and December 2015, 2,085 underwent LDLT Japan. There were 763 male 1,322 female recipients a mean age 5.9 years body weight 18.6 kg. The 1-, 5-, 10-, 15-, 20-year graft survival rates undergoing 90.5%, 90.4%, 84.6%, 82.0%, 79.9%, respectively. mass index, ABO incompatibility, type, recipient age, center experience, transplant era found to be significant predictors overall survival. Adolescent (12 <18 years) was associated significantly worse long-term rate than younger or older ages. We conclude safe effective treatment modality does not compromise donors. optimum timing crucial successful outcome, early referral can improve short-term BA. Further investigation major cause death transplanted essential, especially among adolescents
