ABSTRACT
Abstract Introduction The PREVENT AF I study demonstrated that prophylactic pulmonary vein isolation (PVI) in patients with pure typical atrial flutter (AFL) resulted substantial reduction of new‐onset fibrillation (AF) during 1‐year follow‐up as assessed by continuous implantable cardiac monitor (ICM). objective this was to assess 3‐year outcomes. Methods and results Fifty documented AFL were randomized either cavotricuspid isthmus (CTI) ablation alone (n = 25) or CTI concomitant PVI 25). primary endpoint the occurrence any tachyarrhythmia monthly burden exceeding 0.5% on ICM. At end 3 years, freedom from 48% (95% confidence interval [CI]: 32–72%) plus group compared 20% CI: 9–44%) CTI‐only (P 0.01). Freedom redo procedures also higher: 92% 82–100%) versus 68% 52–89%), respectively 0.027). favored combined group: 6.2% 16.8% 0.03). In group, 12 (48%) hospitalized 4 (16%) + Two developed stroke no serious adverse events group. Conclusion Prophylactic only a significant long‐term ICM, consequent hospitalizations need perform repeat for AF.
