Proton MR Spectroscopy in Patients with Structural MRI‐Negative Temporal Lobe Epilepsy

ABSTRACT
With conventional magnetic resonance imaging (MRI), 20-30% of patients with temporal lobe epilepsy (TLE) have negative pathological MRI findings. Further investigations the role spectroscopy (MRS) in pre-surgical evaluation MRI-negative TLE are important to avoid intracranial EEG recording and better understand mechanism epileptogenic process. This study aimed compare measurements N-acetylaspartate (NAA), creatine (Cr), choline (Cho) hippocampi normal subjects.Twenty 10 age-matched healthy control subjects underwent MRS. The concentrations NAA, Cr, Cho ratios NAA/Cr NAA/(Cr+Cho) were measured. Seven these 20 also surgical treatment for TLE. Their results outcomes evaluated.In ipsilateral seizure side, significantly decreased compared contralateral side hippocampi. There was no significant difference between from who resection indicated mild moderate gliosis minimal loss neurons. Five seizure-free during follow-up period 9- 47 months (mean 27.7 months).In TLE, reductions may help lateralize localize zone.
