ABSTRACT
Surgical intervention for HB with tumor thrombi extending into the IVC and RA might requires careful planning of surgical procedures, including vascular reconstruction extracorporeal circulation. We herein report a successful case LDLT atrial extension thrombus by circulation transdiaphragmatic approach. The patient was 5-year-old boy PRETEXT IV that extended RA. After 4 cycles chemotherapy resection bilateral lung metastases, size primary decreased. As from LHV to had decreased but still present, we performed thrombectomy. central part diaphragm sagittally incised expose suprahepatic Venovenous bypass achieved right femoral vein IMV En bloc native liver then performed. HV anastomosis made between newly created orifice on graft LHV. duration 10 hours 44 minutes (extracorporeal time: 78 minutes). Pediatric LT under is feasible option allows expansion indications transplantation children unresectable tumors.
