A follow up model for patients with atrophic chronic gastritis and intestinal metaplasia

ABSTRACT
To devise a follow up model for patients with gastric cancer associated lesions, such as atrophic chronic gastritis (ACG) and intestinal metaplasia (IM).Cohort study of 144 patients, followed minimum one year, in whom at least two upper gastrointestinal endoscopic biopsies flat mucosa provided diagnosis ACG, IM, or low grade dysplasia (LGD).Of those diagnosed ACG first biopsy (entry biopsy), 12% progressed to LGD outcome biopsy, did 8% type I 38% II III 32% LGD. Type IM entry independently predicted progression cancer. had higher rate than which showed an indolent behaviour similar ACG. Patients were risk development dysplasia, 7% high (HGD), whereas no cases I/II HGD during the three years.Patients could possibly be allocated different management schedules, based on differences proportion HGD. Less intensive (two/three yearly "serological evaluation" (pepsinogen)) may suit IM. benefit from six 12 monthly improved examination (magnification chromoendoscopy).
