ABSTRACT
In patients with chronic coronary artery disease (CAD) and left ventricular dysfunction, flow/metabolic studies of the myocardium positron emission tomography (PET) are able to distinguish viable but dysfunctional from irreversible ischemic injury scar tissue. this study, PET findings blood flow metabolism in chronically hypoperfused were correlated histology.We studied 33 suffering CAD. each patient, myocardial measured 1 or 2 days before revascularization. During surgery, transmural biopsies taken anterior wall planimetrically scored for degree myolysis (sarcomere loss). The amount connective tissue was calculated using morphometric techniques. Contrast ventriculography demonstrated abnormal motion 23 patients. Fourteen a mismatch pattern (decreased preserved metabolism) biopsy region after quantitative analysis data showed 11 +/- 6 vol% fibrosis 25 13% cells sarcomere loss. space formerly occupied by sarcomeres mainly replaced glycogen mitochondria. A significant improvement noted 3 months surgery. Nine match (concordant flow/metabolism defects). revealed 35 25% 24 15% glycogen-storing cells. 10 normal 8 4% 12 8% glycogen-accumulating cells.It can be concluded that areas impaired show extensive fibrosis. Regions reduced FDG metabolism, however, contain predominantly these regions, recovery is found Cells contractile material increased content impairment also present severe stenosis vessel.
