ABSTRACT
Background and Purpose— The causes of death patients with cerebral venous thrombosis (CVT) have not been systematically addressed in previous studies. We aimed to analyze the predictors during acute phase CVT International Study on Cerebral Vein Dural Sinus Thrombosis (ISCVT) identify preventable or treatable causes. Methods— ISCVT is a multinational, prospective, observational study including 624 occurring between May 1998 2001, which 27 (4.3%) died phase, 21 (3.4%) within 30 days from symptom onset. Inclusion forms questionnaire assessing were analyzed. A logistic regression analysis was performed onset CVT. Results— Median time symptoms 13 diagnosis death, 5 days. Causes mainly transtentorial herniation due unilateral focal mass effect (10 patients) diffuse edema multiple parenchymal lesions patients). Independent coma (odds ratio [OR], 8.8; 95% confidence interval [CI], 2.8 27.7), mental disturbance (OR, 2.5; CI 0.9 7.3), deep 8.5; CI, 2.6 27.8), right intracerebral hemorrhage 3.4; 1.1 10.6), posterior fossa lesion 6.5; 1.3 31.7). Worsening de novo deficits increased risk death. Conclusions— main neurologic, most frequent mechanism being herniation.
