Prospective Randomized Trial of Bilateral Carotid Endarterectomies

ABSTRACT
Although several studies have compared the results of carotid endarterectomy (CEA) with primary closure (PC) versus patch closure, none outcome bilateral CEAs PC performed on same patient. This prospective randomized study compares clinical and incidence recurrent stenosis (>/=80%) for CEA in patients CEAs.This includes 74 one side patching other. Patients were to sequential operative treatment either patching/PC or PC/patching. Postoperative duplex ultrasounds follow-up done at 1, 6, 12 months every year thereafter. A Kaplan-Meier analysis was used estimate risk significant restenosis (>/=80%).Demographic characteristics mean diameter internal artery similar both patching. The 29 (range, 6 65 months). ipsilateral stroke 4% 0% had a significantly higher neurological complications (transient ischemic attacks combined) than (12% 1%; P=0.02). Operative mortality 0%. (22% P<0.003) total occlusion (8% 0%; P=0.04) Restenoses necessitating repeated also (14%) (1%; P=0.01). showed that better cumulative patency rate (P<0.01). freedom from 24 75% 98% patching.Patch is less likely cause stroke, transient attacks, stenosis. unilateral may suggest local factors play more role systemic etiology
