ABSTRACT
Hypothesis: To investigate the prognostic role of left ventricular hypertrophy (LVH) regression regarding incidence cardiovascular diseasein essential hypertension. Methods: We prospectively followed up for a median period 3.8 years 1226 hypertensives (mean age 57.8 years, baseline office BP=143.6/89.3mmHg). All subjects visited periodically outpatient hypertensive unit our institution and BP at follow was calculated based on measurements last 3 visits. Echocardiographic evaluation determination metabolic profile creatinine levels performed entry up. LVH defined as LV mass index ≥116g/m2 in men ≥96g/m2 women. Endpoint interest stroke, coronary artery disease their composite. Results: At end composite end-point 4.0% (17 patients with 34 CAD, 2 both). According to presence (20.2%) follow-up (15.9%) were divided two groups: normal both examinations or (n=1031, 84.1%, group 1) (n = 195, 2). Hypertensives compared those 1 older (by 6.3 p<0.001), more frequently females 19%, p<0.001) had greater duration hypertension 2.6 increased number antihypertensive drugs 0.6, pulse pressure 5mmHg, body 0.8kg/m2, p=0.024), glucose 7.4 mg/dl, decreased clearance 10.5 ml/min/, p<0.001).Survival analysis revealed that without (group 2) exhibited significantly higher rates stroke (5.1% vs. 0.7%, log rank (7.7% 3.3%, p=0.020). Conclusions: Lack is accompanied by combined hypertensives.
