ABSTRACT
Background: Hypertensive emergencies (HE) account for 3% of emergency room hospitalizations and it is estimated that 1-2% America’s 50 million hypertensive patients will have at least one occurrence HE in their lifetime. Recent studies shown cardiac troponin-I elevated a third with HE, however the pathophysiology significance this elevation still being explored. We tried to look different factors influence decision do ischemic work up HE. Method: Patients admitted Albert Einstein Medical Center from 01/01/2005 08/30/2014 diagnosis were included retrospective study. divided into two groups: those who had either catheterization or stress test, no up. Demographic clinical variables collected. A cumulative risk score coronary artery disease was calculated by assigning 1 point each factor. Results: There total 187 which only 37 20 out chest pain. Of 3 found significant disease. Comparisons between demographics predictors both groups are table. Multivariate analysis shows pain independently predict more likely receive an (OR=4.7; 95% CI 1.6 7.6; p=0.001). Conclusion: In setting our study single factor predicts Elevated troponins EKG changes alone not perform This may reflect pattern practice center. Large multi-center might be needed explore further also assess outcomes making process.
