ABSTRACT
Introduction: Fewer than 1/4th of US youth meet physical activity guidelines, leading to fitness-related cardiovascular disease disparities tracking into adulthood. Neighborhood opportunity may serve as a critical modifiable factor reduce health-related disparities. The present analysis uses the Child Opportunity Index (COI), comprehensive measure children’s neighborhood opportunity, examine associations between context and multiple fitness outcomes. Methods: Data were drawn from NYC FITNESSGRAM (n = 300,000), comprising data New York City (NYC) public school (grades K-12). Generalized linear mixed models run estimate overall sex-stratified COI individual indicators (greenspace, healthy food, walkability, commute time) (body mass index (BMI) percentiles, curl-ups, push-ups, PACER, sit reach). Geographically weighted regression (GWR) explored spatial variation COI-fitness across NYC. Results: Overall was associated with improved outcomes, strongest magnitude effects aerobic capacity muscular strength endurance measures (PACER: β: 0.15, 95% CI: 0.14, curl-ups: 0.13, 0.12, 1490.13, push-ups: 0.13). Greenspace positively (curl-ups: 1.13, CI:0.76, 1.50; 1.53, 1.21, 1.84), walkability commuting time 2.11, 1.72, 2.50; 2.03, 1.87, 2.19, respectively). Stratified showed strengthened among girls versus boys.
