ABSTRACT
Background: Uncontrolled blood pressure (BP) is a leading preventable cause of death that remains common in the US population despite availability effective medications. New technology and program innovation has high potential to improve BP but may be expensive burdensome for patients, clinicians, health systems, payers not produce desired results or reduce existing disparities control. Methods Results: The PCORnet Blood Pressure Control Laboratory platform designed enable national surveillance facilitate quality improvement comparative effectiveness research. uses PCORnet, National Patient-Centered Clinical Research Network, engagement systems collection electronic record data, Eureka Platform eConsent patient-reported outcomes mHealth data from wearable devices smartphones. Three demonstration projects are underway: track will conduct control related clinical processes by measuring theory-derived pragmatic metrics using with focus on tracking over time; MAP cluster-randomized trial comparing 2 versions program; Home an individual patient-level randomized smartphone-linked versus standard home monitoring. Thus far, Track collected 826 000 eligible patients hypertension who completed ≈3.1 million ambulatory visits. Preliminary demonstrate substantial room (<140/90 mm Hg), which was 58% overall, relevant For example, only 12% measurement during visit received order new antihypertensive medication. Conclusions: reusable efficient research; forthcoming.
