ABSTRACT
Background: Cardiovascular disease (CVD) risk factor control is critical to reducing CVD and adverse outcomes. Recent studies from CDC revealed that American adults who smoke decreased 20.9% in 2005 19.3% 2010.In addition, prevalence of obesity appears be slowing or leveling off. However,minority low-income populations face barriers literacy poverty blocking access the benefits efficacious strategies these factors for CVD. Federally Qualified Health centers (FQHCs {Safety-Net Clinics}) provide care low income medically underserved populations. Objectives: 1) Determine if obese diabetic/ patients FQHCs have community resources weight loss. 2) smoking rates accessibility cessation programs Methods: This study part Office- Guidelines Applied Practice (Office-GAP) project, designed improve secondary prevention heart diabetic Michigan. We analyzed 242 participated Office-GAP group visits (September 2010-December 2012). Patients completed a health survey at baseline included demographic characteristics, comorbidity, status, weight, having loss nutrition counseling, programs. Chart abstraction was performed relevant data by trained research assistants. Descriptive analysis on cross sectional obtained. Results: Among enrolled 53.71% (130 242) were (BMI > 30). Mean age=53.69; male=33.07% (43 130); white=47.69% (62 130). Analysis 55.85% (105 188) diabetics, 57.06% (109 191) hypertensives 57.57% (19 33) MI obese. Only 57.42% (58 101) 62.5% (40 64) said they counseling respectively.
