ABSTRACT
Background and Purpose- Using a novel study design with virtual comparators based on predictive modeling, we investigated whether next-generation mechanical thrombectomy devices improve outcomes in patients ischemic stroke. We hypothesized that this new shows system is superior to intravenous tPA (tissue-type plasminogen activator) therapy (IVT) alone. Methods- ERASER (Eric Acute Stroke Recanalization) was an investigator-initiated, prospective, multicenter, single-arm (virtual 2-arm) evaluated the effectiveness of recanalization device together specific intermediate catheter (Embolus Retriever Interlinked Cages/SOFIA, Microvention) stroke internal carotid artery or middle cerebral occlusions. The primary end point volume saved tissue. Volume tissue defined as difference actual infarct brain predicted develop infarction using machine learning model data from patients. Results- Eighty-one were enrolled. median patient age 71 years (interquartile range, 61-77). National Institutes Health Scale score 14 12-18). smaller than by model, 50 mL 19-103; P<0.0001). Good clinical outcome (modified Rankin Scale, 0-2 at 90 days) observed 48 out 69 (70%). rate (Thrombolysis Cerebral Infarction 2b/3) 95%. Conclusions- first analytics enabling intraindividual comparisons. method resulted infarcts after alone showed high good outcome. comparisons promising for further application testing neurovascular arena.
