ABSTRACT
The weaning process concerns all patients receiving mechanical ventilation. A previous classification into simple, prolonged, and difficult ignored failure presupposed the use of spontaneous breathing trials.To describe process, defined as starting with any attempt at separation from ventilation its prognosis, according to a new operational working for under ventilation.This was multinational prospective multicenter observational study over 3 months in 36 intensive care units, daily collection modalities. Pragmatic definitions success allowed us allocate four groups.A total 2,729 were enrolled. Although half them could not be classified using definition, 99% entered groups on basis our definition follows: 24% never started 57% had less than 24 hours (group 1), 10% more 1 day week 2), 9% prolonged duration or 3). Duration ventilation, unit stay, mortality (6, 17, 29% three groups, respectively) significantly increased one group next. unadjusted risk dying 19% after first 37% 10 days.A allows categorize situations.
