ABSTRACT
Chronic obstructive pulmonary disease (COPD) is one of the main causes morbidity and mortality globally. In Trondheim in 2008 an integrated care model (COPD-Home) consisting education program, self-management plan, home visits a call centre for patient support communication was developed.
