ABSTRACT
The reasons for reduced exercise capacity in diabetes mellitus (DM) remains incompletely understood, although diastolic dysfunction and diabetic cardiomyopathy are often favored explanations. However, there is a paucity of literature detailing cardiac function reserve during incremental to evaluate its significance contribution. We sought determine associations between comprehensive measures maximal oxygen consumption ( $$\dot{V}O_{2}$$ peak), with the hypothesis that reduction would be associated co-morbidities sedentary behavior rather than itself. This case–control study involved 60 subjects [20 type 1 DM (T1DM), 20 T2DM, 10 healthy controls age/sex-matched each subtype] performing cardiopulmonary testing bicycle ergometer echocardiography studies. Measures biventricular were assessed intensity. T2DM middle-aged (52 ± 11 years) mean diagnosis 12 7 years modest glycemic control (HbA1c 57 mmol/mol). T1DM participants younger (35 8 years), 19 year history suboptimal 65 16 Participants heavier their (body mass index 29.3 3.4 kg/m2 vs. 24.7 2.9, P = 0.001), performed less (10 28 30 MET hours/week, 0.031) had lower peak 26 6 38 ml/min/kg, < 0.0001). These differences not systolic or left ventricular (LV) at rest exercise. There was no difference weight, participation as compared controls. After accounting age, sex body surface area multivariate analysis, significant positive predictors size (LV end-diastolic volume, LVEDV) estimated MET-hours, while negative predictor. combined factors accounted 80% variance (P Exercise relative matched controls, whereas preserved T1DM. evidence sub-clinical but, rather, an association impaired capacity, small LV volumes behavior.
