ABSTRACT
Abstract Background Covid-19 has the respiratory tract as main target of infection, and patients present mainly dyspnea, pneumonia, dry cough, fever. Nevertheless, organs outside had been reported in recent studies, including gastrointestinal liver. The host innate immune system recognizes pathogen-associated molecular patterns (PAMPs) through their pattern recognition receptor (PRRs). Toll-like 7 (TLR-7) is a recognizing ssRNA (SARS-CoV-2 an ssRNA). Polymorphisms are characterized by two or more alternative forms distinct phenotype same population. tlrs genes can negatively influence response to infectious diseases. There several references literature non-synonymous single nucleotide (rs) polymorphisms related genes. Some them important for immunity, rs 179008 ( tlr-7 ), rs3775291 tlr3 rs8177374 tir domain-containing adaptor protein, tirap rs1024611 monocyte chemoattractant protein-1, mcp-1 ) rs61942233 2 ′ -5 -oligoadenylate synthase-3, oas-3 ). Case presentation We identified 5-year-old-male child with symptoms fever presenting acholic stool jaundice, who was positive SARS-CoV-2 IgM, IgA, IgG Gln11Leu . presented high levels aspartate aminotransferase, alanine bilirubin, C-reactive D-dimer, gamma-glutamyl transferase, alkaline phosphatase, negative serological tests hepatitis A, B, C, E, HIV 1 2, herpes virus, cytomegalovirus, Epstein–Barr RTqPCR Influenza A RSV SARS-CoV-2. also investigated other SNPs tlr-3 (rs3775291) , (rs8177374) (rs1024611) (rs61942233) genes, no mutation detected After interview child's caregivers, any possible accidental ingestion drugs hepatotoxic substances ruled out. Conclusion To our knowledge, this first report caused male that 179008, which could impair efficient initial response. knowledge patient's deficiency improve treatment correct specific medications.
