An unusual presentation of anomalous left coronary artery from the pulmonary artery (ALCAPA) syndrome in a 70-year-old man: a case report

ABSTRACT
We present a rare case of anomalous origin the left coronary artery from pulmonary syndrome in an elderly man, and we describe computed tomographic angiographic imaging findings to improve diagnostic confidence for evaluation this uncommon anomaly.A 70-year-old Caucasian man came our hospital with slight limitation physical activity (New York Heart Association class II). He was asymptomatic angina, syncope, palpitations. Cardiac magnetic resonance performed after echocardiography because hypertrophic cardiomyopathy suspected; plausible anomaly demonstrated as collateral evidence. Subsequently, angiography showed artery; vessels appeared markedly dilated tortuous. Dilated intercoronary along epicardial surface heart bronchial arteries, corresponding pathways, were observed. Left ventricular hypertrophy, delayed subendocardial enhancement, mitral insufficiency better evaluated on images. Invasive confirmed main findings. Given patient's age clinical performance, surveillance medical management considered appropriate, surgical repair avoided.Confidence anatomic pattern significance condition is necessary cardiac ability. In patient, proved be reliable approach, superior invasive terms performance patient safety.
