ABSTRACT
PURPOSE Resection of solitary metastases from renal cell carcinoma (RCC) is associated with a 5-year survival rate 35% to 50%. Selection criteria are not well defined. PATIENTS AND METHODS We retrospectively analyzed our experience 278 patients recurrent RCC 1980 1993. RESULTS One hundred forty-one underwent curative metastectomy for their first recurrence (44% overall [OS] rate), 70 noncurative surgery (14% OS and 67 were treated nonsurgically (11% rate). Favorable features disease-free interval (DFI) greater than 12 months versus or less (55% v 9% rate; P < .0001), multiple sites (54% 29% .001), age younger 60 years (49% .05). Among 94 metastasis, lung (n = 50; 54% rate) was more favorable brain 11; 18% Survival rates after resection second third different compared initial (46% 44%, respectively, 43% rates; nonsignificant). predictors by multivariate analysis included single site recurrence, long DFI, metachronous presentation recurrence. CONCLUSION Selected who can undergo disease have good opportunity long-term survival, particularly those and/or DFI.
