ABSTRACT
Purpose: To estimate the risk of secondary leukemia as a function dose epipodophyllotoxins and anthracyclines. Methods: We conducted case-control study or myelodysplasia after solid tumor in childhood within Société Française d’Oncologie Pédiatrique, including 61 patients with matched 196 controls. The characteristics first cancer, patient’s family history treatment (type, cumulative chemotherapy, schedule etoposide administration, radiation delivered to active bone marrow) were compared two groups. Results: Only factors found increase multivariate analysis, namely, type tumor, an excess Hodgkin’s disease (relative 6.4; 95% confidence interval [CI], 1.6 24) osteosarcoma 5; CI, 1.3 19), exposure increased regularly etoposide. In summary, who received between 1.2 6 g/m 2 more than 170 mg/m anthracyclines had seven-fold higher (95% 2.6 19) lower doses none these drugs. was multiplied by 197 19 2,058). not alkylating agents radiotherapy. Conclusion: Both leukemia. current challenge is minimize mutagenic effects drugs diminishing without losing therapeutic benefits.
