ABSTRACT
Uncontrolled studies suggest that chemoradiation has similar efficacy as surgery for esophageal cancer. Therefore, a randomized trial was carried out to compare, in responders only, alone with followed by patients locally advanced tumors.Eligible had operable T3N0-1M0 thoracic Patients received two cycles of fluorouracil (FU) and cisplatin (days 1 5 22 26) either conventional (46 Gy 4.5 weeks) or split-course (15 Gy, days concomitant radiotherapy. response no contraindication treatment were randomly assigned (arm A) continuation B; three FU/cisplatin [20 Gy] [15 radiotherapy). Chemoradiation considered equivalent if the difference 2-year survival rate less than 10%.Of 444 eligible patients, 259 assigned; 230 (88.8%) epidermoid cancer, 29 (11.2%) glandular carcinoma. Two-year 34% arm A versus 40% B (hazard ratio v = 0.90; adjusted P .44). Median time 17.7 months compared 19.3 B. local control 66.4% 57.0% B, stents required (5% 32% < .001). The 3-month mortality 9.3% 0.8% (P .002). Cumulative hospital stay 68 52 .02).Our data that, cancers, especially epidermoid, who respond chemoradiation, there is benefit addition after additional chemoradiation.
