ABSTRACT
8152 Background: The management of the cancer patients frequently relies on ability to deliver a variety intravenous agents over prolonged period. A central venous catheter (CVC) currently represents most adopted line for undergoing infusional chemotherapy and/or high-dose with concomitant blood stem cell transplantation and parenteral nutrition. aim this study was explore feasibility safety CVC insertion under ultrasound (US) control. US offers advantage real-time, multiplanar imaging as well Doppler analysis, which makes it very practical improves by allowing operator identify major vessels. Methods: patient is placed in Trendelenburg’s position. After sterilization, local anaesthesia applied 7.5 MHz puncturing probe right supraclavicular site 16 gauge needle advanced into last portion internal jugular vein nearly innominate US-control; after guidewire (Secalon Seldy, Becton Dickinson) superior vena cava until atrium. At end procedure an upright chest X-ray carried out confirm position rule pneumothorax. Results: From June 2001 2004 has been performed 600 times 519 consecutive patients: 350 solid tumors 169 hematological malignancies. Some underwent more then one time along their clinical course (complete remission, relapse, collection transplantation, nutrition). successful 595/600 (99.1%), mean permanence 151 days (range 7–701 days). Four cases deep thrombosis were detected successfully treated low molecular weight heparin. Infections episodes 1.7/1000 use, antibiotics. No increase infections registered setting. pneumothorax occurred. Conclusions: This safe, cheap high accuracy success rate, above all US-guidance avoids significant financial relationships disclose.
