ABSTRACT
The neoplastic Hodgkin Reed-Sternberg (HRS) cells in classical Hodgkin's lymphoma (cHL) are derived from B cells. frequency of HLA class II downregulation and its effect on prognosis unknown.Immunohistochemistry results for were evaluated 292 primary cHL patients a population-based approach. Patients diagnosed between 1989 2000 the northern part Netherlands. Median age at diagnosis was 38 years (range, 8 to 88 years); 63% had Ann Arbor stage I or II, 24% III, 13% IV disease. follow-up 7.1 years. For 168 patients, genotype data available.Lack cell-surface expression HRS observed 41.4% more common with extranodal disease, Epstein-Barr virus-negative I-negative Alleles three microsatellite markers region associated presence absence protein expression. In univariate analyses, lack coincided adverse outcome (5-years failure free survival [FFS], 67% v 85%; P = .001; 5-years sex matched relative (RS), 80% 90%; .027). This remained multivariate analyses FFS hazard ratio 2.40 (95% CI, 1.45 3.98) RS excess risk death 2.55 1.22 5.31).Lack membranous by diagnostic lymph node specimens is an independent prognostic factor cHL.
