68 Background: Survivorship care plans (SCPs) are considered an important tool in survivorship care. SCP should include treatment summaries and follow up intended to improve Despite the institute of medicine (IOM) recommendation regarding SCPs, uptake has been low community cancer centers. The safety net hospitals face additional challenges SCPs including patient who may have health literacy limited English proficiency. In addition, resources personnel constraints can be barriers for implementation SCPs. Methods: Zuckerberg San Francisco General Hospital is a hospital City County Francisco. We created breast survivors, as it our largest group survivors.
