ABSTRACT
68 Background: Survivorship care plans (SCPs) are considered an important tool in survivorship care. SCP should include treatment summaries and follow up intended to improve Despite the institute of medicine (IOM) recommendation regarding SCPs, uptake has been low community cancer centers. The safety net hospitals face additional challenges SCPs including patient who may have health literacy limited English proficiency. In addition, resources personnel constraints can be barriers for implementation SCPs. Methods: Zuckerberg San Francisco General Hospital is a hospital City County Francisco. We created breast survivors, as it our largest group survivors. Patients, were within 5 years completion with curative intent candidates receiving used ASCO template create plan. was discussed focus groups modified match needs patients. committee advisory board evaluated prior pilot clinic. piloted initially Spanish assess feedback from clinic providers.
