529 Background: Late relapses (LR) of non-seminomatous germ cell tumours (NSGCT), defined as those occurring after a disease free interval 2 years, are increasingly recognised. We reviewed the features LR in NSGCT within tertiary referral testicular cancer service. Methods: 3,064 patients (pts) were referred to testis multi-disciplinary team (January 2005 Dec 2017). Pts who experienced where initial pathology demonstrated identified. Data their original and late presentation management was from patient electronic records. Results: Of pts, 101 (3.3%) had LR, with 43 pts (43%) relapsing 10 years. 36 symptomatic 39 raised markers (AFP 29, HCG 9, both 1). Table 1 shows stage at time relapse. As regards treatment before relapse, 13 CS1 received prior chemotherapy (Cht) (8 adjuvant 5 Cht for early relapse). 59/60 CS2/3 primary 41 post retroperitoneal lymph node dissection(PC-RPLND (bilateral template 12). 20 these men PC-RPLND retroperitoneum (6 bilateral template) Patient relapse is summarised table.Time recurrence longer (p<0.001). 84 surgical procedures [performed 82 patients; histology TD 43, Yolk sac 14, De-differentiated 7 viable GCT 12, benign 8. To date 22 have died, relapsed multiple sites. Conclusions: frequently occurs an extended typically earlier compared higher stages. Aggressive surgery +/- can cure most patients. [Table: see text]
