ABSTRACT
10622 Background: Access to genetic testing and risk stratification is primarily gated by referrals from non-genetics providers for patients who meet criteria. The majority of Americans seek care at community hospitals that are closer home; faced with the challenge how democratize access precision oncology cancer prevention. Research has shown establishment such protocols challenging, requiring organizational buy-in, capital, resources, involvement multiple levels clinical nonclinical stakeholders. Here we highlight Sarah Cannon, Cancer Institute HCA Healthcare, their programmatic approach using a digital platform offered universally all undergoing screening mammography. Methods: From February November in 2022, fourteen sites were assessed have appropriate provider facility capabilities support implement CARE program. web-based risk-stratification tool sent individuals before standard ambulatory appointments assess lifetime breast based on Tyrer-Cuzick (version 8.0) algorithm National Comprehensive Network (NCCN) Testing criteria included hereditary breast, ovarian, pancreatic, prostate cancers, Lynch syndrome, familial adenomatous polyposis (FAP). Retrospective data was pulled seen between 2022 through January 2023. outcome measures percentage completed risk-assessment, met criteria, pursued germline testing, received positive result, and/or had ≥20%. Results: During 10 months 14 total 25,671 invited assessment program; 19,277 (75.1%) assessment. Overall, 5,070 (26.3%) 1,536 ≥20% cancer, 34% identified as elevated risk. 23.4% (1,187) those meeting opted proceed testing. An additional 126 did not Of 1,313 tests, 96 (7%) test results. Fifty-four (56%) results an impact medical management Conclusions: Cannon Healthcare successfully begun implementing comprehensive, program hospital setting provide standardized genomic stratification. By identifying potential increased early detection can be observed scalable sustainable
