ABSTRACT
e13810 Background: With the 21st Century Cures Act of 2016, rapid and complete access to medical records including radiology reports is mandated. Patients have expressed that an unmet need improving clarity in results with more structured information avoids technical jargon. RECIST routinely applied clinical trials assess treatment response but challenging apply routine practice. A standardized Oncologic Response (OR) lexicon was developed categorize progression directly from reports, using ordinal scale. This study aims compare rates OR categories setting three phase 2 trials. Methods: were retrospectively reviewed for real world by a radiologist blinded data. Clinical generated during trial period categorized report impression text according (OR-0: no evidence disease, OR-1: unequivocal decrease, OR-2: slight OR-3: unchanged, OR-4: increase, OR-5: increase). Best (CR) or partial (PR), stable disease (SD), (PD) compared best (OR-0/1, OR-2/3/4, OR-5, respectively). Time failure (TTF) overall survival (OS) each category calculated Kaplan-Meier methodology. Results: Ninety-one patients reviewed. based on v1.1 CR 4 (4%), PR 38 (42%), SD 44 (48%), PD 5 (6%). Using OR-categories, 1 patient (1%) as OR-0, 46 (51%) OR-1, 34 (37%) 10 (11%) OR-5. Concordance between across (CR PR; SD; PD) OR-RADS (0-1; 2/3/4; 5) 73.6%. The majority discordant cases (50%, n=12/24) OR-1 categories. Median TTF similar categories: CR/PR: 21.8, SD: 6.9, PD: 1.0 months (p<0.0001) OR-0/1: 21.5, OR-2/3/4: 7.0, OR-5 1.1 (p<0.0001).
