ABSTRACT
32 Background: The prevalence of early-onset colorectal cancers (EOCRC) is rising globally. Disparities in social determinants health can delay access to treatment and impacting outcomes. This study aims analyze factors influencing time from diagnosis among EOCRC patients a racially diverse population, considering tumor sociodemographics. Methods: We analyzed data diagnosed between 2000 2022 at Queen’s Medical Center Honolulu, Hawaii. Time for Asians, Whites, Native Hawaiian or Other Pacific Islanders (NHOPI) was assessed using the Kaplan-Meier Method. Cox proportional hazards regression models were used identify predictors, adjusting clinical pathological factors. Results: A total 379 analyzed. NHOPI more like be underinsured compared Whites Asians. In unadjusted model, underinsured/uninsured status, older age, unknown stage, left sided, rectal locations associated with delayed treatment. After adjustment, location remained significant predictors Conclusions: are significantly cancer While age statistically significant, its impact appears minimal. need multidisplinary approach additional preoperative staging may explain delays even after adjustment. Improving accessibility reducing delays, especially patients, important.
