ABSTRACT
The appropriateness of current reference ranges for thyroid function testing in older adults has been questioned. This study aimed to determine the relationship between tests within euthyroid range and adverse outcomes not taking medication. US community-dwelling years (n = 2843) enrolled onto Cardiovascular Health Study with TSH, free T4 (FT4), total T3 concentrations range. Incidence atrial fibrillation, coronary heart disease, failure, hip fracture, dementia, all-cause death were measured. No departures from linearity detected. Higher TSH was negatively associated (P .03) higher FT4 positively .007) mortality. fibrillation < .001) failure .004). Compared first quartile, individuals fourth quartile had a 9.6 per 1000 person-year lower incidence dementia .05) those incidences mortality (11.0, 8.0, 7.8, 14.3 person-years, respectively, all P .05). Total any outcome. are risk multiple events people, including suggests tolerance hormone levels this age group. Clinical trials needed evaluate risk-benefit profile new thresholds initiating treatment optimal target replacement people.
