ABSTRACT
Background It is important to achieve the low-density lipoprotein cholesterol (LDL-C) goal recommended by clinical guidelines in managing risk of cardiovascular (CV) events, however, current management LDL-C actual settings suboptimal. We examined level among patients with dyslipidemia against 2015 Korean guidelines, crude rates CV events based on achievement, and factors associated achievement. Methods This was a retrospective cohort study using National Health Insurance Service–National Screening Cohort (NHIS-HEALS) database from 2006 2013. Patients who had health examination measurement between January 1, 2007, December 31, 2011 were identified. required have at least one diagnosis during 1 year before index date, defined as first date measurement. The used measure achievement level. Crude event calculated for total individual number divided person-years (PYs) follow-up period. included acute coronary syndrome, ischemic stroke, peripheral artery disease, death, all-cause death. Factors assessed logistic regression. Results In NHIS-HEALS database, 69,942 met eligibility criteria: 36.7%, 22.5%, 20.1%, 20.6% very high-, moderate-, low-risk groups respectively, guidelines. Approximately half (47.6%) achieved their goal, but substantially different across levels (17.6%, 47.2%, 66.9%, 82.4% groups, respectively; P<0.0001). rate period non-achievers higher than that achievers (24.35/100 PYs vs. 11.93/100 PYs; significantly patient characteristics, including age, sex, body mass index, lipid-modifying therapy, Conclusions South Korea, high or did not showed goal. strategies should be highlighted are less likely such female, overweight obese patients, adherent statin, risk.
