Out-of-Pocket and Informal Payment Before and After the Health Transformation Plan in Iran: Evidence from Hospitals Located in Kurdistan, Iran

ABSTRACT
Background One of the objectives health transformation plan (HTP) in Iran is to reduce out-of-pocket (OOP) payments for inpatient services and eradicate informal payments. The HTP has three phases: first phase (launched May 5, 2014) focused on reducing OOP services; second 22, primary healthcare (PHC) third utilizes an updated relative value units September 29, elimination This aim this study was determine frequency cash physicians before after Kurdistan province, Iran. Methods quasi-experimental used multistage sampling method select evaluate 265 patients discharged from hospitals province. covered 3 phases (before HTP, first, HTP). Part data collected using a hospital information system form rest were questionnaire. Data analyzed Fisher exact test, logistic regression, independent samples ttest. Results mean phases, respectively, US$59.4, US$17.6, US$14.3 affiliated Ministry Health Medical Education (MoHME), US$39.6, US$33.7, US$13.7 Social Security Organization (SSO), US$153.3, US$188.7, US$66.4 private hospitals. In SSO MoHME there significant difference between (P < .05). percentage MoHME, SSO, sector, 4.5%, 8.1%, 12.5% 0.0%, 7.1%, 10.0% phase.
