Purpose: Event reporting is an important tool by which a health care system can recognize what types of errors are occurring and identify ways to improve patient safety. However, national reporting data show that a minority of event reports are made by physicians. Patient safety experts have targeted low physician reporting as an area for improvement on both national and local levels.
